Wanda Smythe’s Mobile Phlebotomy Services is responsible for the drawing and delivery of laboratory specimens. WSMPS does not collect any specimens without a physician order or a request from a laboratory. AMPS is not allowed to give any medical advice, treat, or diagnose anyone.
Wanda Smythe’s Mobile Phlebotomy Services (WSMPS) needs patient information, copy of order and copy of insurance card (front and back) 48hours before scheduled appointment. If we do not receive the requested information in the time range mentioned, then the scheduled appointment will be considered canceled. This is extremely important so that we have adequate time to prepare for the blood draw and that we perform our job correctly.
Wanda Smythe’s Mobile Phlebotomy Services handles the delivery of all samples to their proper laboratory. If a sample is being collected for a laboratory that needs to be picked up via courier service, WSMPS is not responsible for any delay of arrival to the laboratory, due to weather conditions or any other problems that may occur once the sample has been delivered or picked up by courier service.
Wanda Smythe’s Mobile Phlebotomy Services is not responsible for any errors that may occur at the laboratory. It is the labs responsibility to process specimen once received by AMPS. In the event of a lab error, WSMPS would gladly return to recollect the sample at a discounted rate. Any error made by WSMPS will be corrected by re-drawing at no extra charge; as long as all correct paperwork was submitted adequately prior to appointment.
Any errors in collection caused by Wanda Smythe’s Mobile Phlebotomy Services (drawing the wrong tubes, unable to obtain a satisfactory blood flow to collect the specimen etc.) will not incur a charge to the patient or will be redrawn at no additional charge to the patient.
Wanda Smythe’s Mobile Phlebotomy Services makes every attempt to have contact with the patients prior to the draw. We do not show unless we have made verbal contact with the patient the night before or morning of the scheduled blood draw.
Please be considerate, in the event an appointment needs to be cancelled, Wanda Smythe’s Mobile Phlebotomy Services needs to be given a 24-hour notice. You may reach us by phone or via email or text.
If we arrive at the appointment for the blood draw and are unable to collect the specimen due to circumstances beyond our control – example (the patient refuses, fasting blood draw was scheduled but the patient ate food, the patient did not drink plenty of water, the patient gets taken to the hospital after they have ordered STAT blood work or not available to be drawn etc.) the patient will remain responsible for the full charges of the draw.
Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department. Payment for services provided by Wanda Smythe’s Phlebotomy Services is collected on the day prior to appointment. Wanda Smythe’s Mobile Phlebotomy Services does not bill insurance companies, any claims must be submitted by the client, with no guarantee of reimbursement.
Wanda Smythe’s Mobile Phlebotomy Services is a private pay service. Specimen collection services are provided and billed by Wanda Smythe’s Phlebotomy Services or One Road Many Paths, NFP. Laboratory testing is billed through your insurance. Payments that have not been prepaid will be due at the time of service.
Please be advised that Wanda Smythe’s Mobile Phlebotomy Services is not responsible for any outstanding balances with LabCorp or Quest or other providers and will need to be settled prior to a scheduled home lab draw. The labs have the right to decline specimens of patients with outstanding balances.
Wanda Smythe’s Mobile Phlebotomy Services is an independent convenience service. Wanda Smythe’s Mobile Phlebotomy Services is not affiliated with any laboratories or doctor’s offices. Wanda Smythe’s Mobile Phlebotomy Services does not have the authorization to give discounts for any lab tests that are being collected.
We are fully HIPPA compliant and respect the privacy of clients we serve. Under no circumstances Wanda Smythe’s Mobile Phlebotomy Services will give out client’s personal information to unauthorized personnel. All client’s information is always kept personal and confidential.
Wanda Smythe’s Mobile Phlebotomy Services does not receive any results. In the event that your doctor did not receive results, it is the responsibility of the Doctor office to get in touch with the lab. Most of the time they have an account with the labs.
